Form guidelines

1. Your name, address, phone number, and email are useful for follow-up communication and reporting results. All information will be kept confidential.
2. Specify the reason for the complaint together with facts, circumstances, date, time, and place related to the issue.
3. Include relevant evidence (if any).
4. Provide the name and address of the person being reported (if any).
5. Anonymous complaints will be treated as anonymous notices and processed in accordance with Cabinet Secretariat letter No. นร 026/ว218 dated 25 December 1998 on guidelines for complaints against civil servants.
